Clinical Pharmacist

Key Responsibilities:

• Review prescriptions and medication regimens to ensure the safe, effective, and appropriate use of medicines.
• Participate in the development and implementation of policies and procedures related to safe clinical practice.
• Participate in the development and implementation of new clinical services, guidelines, protocols and clinical pathways.
• Ensure all pharmacy procedures are in accordance with legal and hospital Standard Operating Procedures (SOPs).
• Provide guidance to Pharmacy Technicians (PTs) and Provisionally Registered Pharmacists (PRPs).

Job Requirements:

* Bachelor's Degree in Pharmacy recognized by the Pharmacy Board of Malaysia.

* Registered Pharmacist with a valid Annual Retention Certificate (ARC).

* Minimum 3 to 4 years of hospital pharmacy practice, with relevant clinical pharmacy experience.

* Ability to demonstrate good clinical pharmacy knowledge skills, clinical reasoning and judgement when assessing patient's pharmaceutical needs.

* Passionate and possesses the desire to continually improve knowledge & clinical skills.

* Good interpersonal skills and able to work in a multidisciplinary environment.
